We provide medical card physicals.

At Southwest Missouri Wellness Center, we provide medical card physicals provided you have a qualifying condition.

 

Once your application is approved and your identification card is in hand, your medical card will be mailed to you within 30 days. Our staff is professional, friendly and your privacy is of utmost importance to us.

RESIDENCY REQUIREMENTS
You must be a resident in the state of Missouri.

At this time, the Missouri Department of Health and Human Services (DHHS) is not allowing out of state patients to qualify.

AGE REQUIREMENTS
You must be 18 or older.
QUALIFYING CONDITIONS

Any condition already diagnosed by a doctor may qualify. Some of the common qualifying conditions are depression, PTSD, chronic pain as well as the following:

  1. Cancer
  2. Epilepsy
  3. Glaucoma
  4. Intractable migraines unresponsive to other treatment
  5. A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, Parkinson’s disease, and Tourette’s syndrome
  6. Debilitating psychiatric disorders, including, but not limited to, post-traumatic stress disorder, if diagnosed by a state licensed psychiatrist
  7. Human immunodeficiency virus or acquired immune deficiency syndrome
  8. A chronic medical condition that is normally treated with a prescription medication that could lead to physical or psychological dependence, when a physician determines alternative methods of treatment would serve as a safer alternative to the prescription medication
  9. Any terminal illness
  10. In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C, amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia and wasting syndrome.

Bring supporting medical documentation along with your Missouri ID to your appointment. Proof includes medical records, office visit notes or a letter from your doctor.

A Southwest Missouri Wellness Center physician will review your medical records and perform a physical exam. Starting July 4, 2019, you can apply for an identification card from the Missouri Department of Health and Senior Services (DHSS). Once your application is approved you will receive your identification card within 30 days.

Frequently Asked Questions

Will my registration status be made publicly available?

No. The law prohibits the DHSS, as well as Missouri licensed dispensaries, from making personal identifying information public. Physicians and those employed will be able to verify a patient or caregiver registration.

Will Missouri accept medical cards from other states?

No. You must be a patient in Missouri.

Will minors be allowed to obtain an identification card?

No. You must be 18 years or older and a legal resident of Missouri.

When will I receive my Missouri medical identification card?

Within 30 days of application, if your application is approved.

What will take place at my first visit with Southwest Missouri Wellness Center?

A registered Missouri medical provider will examine you, review medical records, assess your medical history, maintain your patient records and provide follow-up care as needed.

What will I need for my appointment at Southwest Missouri Wellness Center?

You will need a photo ID and medical records that say something about the condition for which you are trying to enter the program.
